during organized and recreational sports.

TO PROVIDE CUSTOM-FITTED MOUTH GUARDS TO THE FIRST 2 TEAMS
THAT RESPOND AS DIRECTED BELOW!
THE CONTEST IS OPEN TO ANY 7th OR 8th GRADE
PATIENT FOR THEMSELF AND THEIR TEAM!

with the team name, school or league name, location and number of players.
The 2 winning teams will be contacted to coordinate appointments for their teams.
